When people say 'Hong Kong' Are they referring to 'Hong Kong Island'?

Yes. People in Hong Kong refers Hong Kong as Hong Kong Island and people from outside Hong Kong refers Hong Kong as a whole and not just the island itself. But regardless of who you are, when you are not in Hong Kong, even if you are in Macau or Shenzhen, when people say Hong Kong, they all refers to the territory as a whole and not just the island itself. So its that easy.

hk is the special administrative zone that include new territories, kowloon, hk island, lantau island and some small islands.

No, it usually referres to the three islands all together, if you are talking about the island, then you say HK island. (besides, most Americans don't know the parts anyway.)

When I am in HK and I say "I go to HK" then I mean HK Island only. When I am out of HK and say "I go to HK" then I mean the HK Territory as a whole.

When I talk to somebody not from HK I would refer to HK Island when I mean HK Island, I wouldn't imply that they know those subtle differences.
